Jenny's office will help you apply for a congratulatory message to be sent to yourself or someone you know who is about to celebrate a significant birthday or wedding anniversary.

Forms are available from Jenny's office. Please make sure you bring along a copy of the birth certificate or a copy of the wedding certificate, whichever is applicable.

If you can't get a copy of these certificates, you will need to obtain a statutory declaration stating the full name of the recipients, their current address and the date of the birthday or wedding anniversary.
